Getting Started
Referrals
We are a referral-based practice. Referrals may be submitted by primary care physicians, specialists, or other healthcare providers. If you are interested in becoming a patient, please contact your provider to initiate a referral.
Scheduling
Appointments are scheduled after referral review and acceptance. Our office will contact you with next steps, including scheduling and any required documentation prior to your visit.
Initial Appointment
New patient appointments typically include a comprehensive evaluation conducted by a licensed clinician. This visit focuses on reviewing your history, current concerns, and determining appropriate next steps for care.
What to Bring
Please be prepared to provide the following at your appointment: Current insurance information, valid identification, list of current medications, relevant medical or psychiatric records (if available)
Important Information
Care is provided on an outpatient basis by appointment only. Our office does not provide emergency or after-hours services. If you are experiencing a medical or psychiatric emergency, please call 911 or go to your nearest emergency room.
